Documentation

In order to better help our students with disabilities, the following information will need to be on file with the Admissions Department:

1. The most current copy of a student's Individualized Education Plan (IEP) or 504 plan with the accommodations listed (preferably from the senior year, but it must include accommodations).
2. A copy of the student's psychological report that is no more than 3 years old.
3. A signed Consent Form.
